1. Patient Responsibilities: a. Provide accurate and complete medical history information. b. Follow medical advice and treatment plans prescribed by our healthcare professionals. c. Notify the clinic of any changes in contact information promptly.
2. Appointments: a. Appointments can be scheduled online, by phone, or in person. b. Cancellations must be made at least 24 hours in advance to avoid cancellation fees. c. Late arrivals may result in rescheduling of appointments at the discretion of the healthcare provider.
3. Confidentiality: a. Patient medical records are confidential and will not be disclosed without the patient’s consent, except as required by law. b. Electronic communication, including email, is not guaranteed to be secure. Patients should avoid sending sensitive information through unsecured channels.
4. SMS Opt-In Agreement
   By opting into our SMS notifications, you consent to receive text messages to the phone number you provided at registration or updated later. These messages may include updates, alerts, and promotional content relevant to our services.
   
   Consent & Charges: You confirm that your consent to receive these messages is not required to purchase any goods or services. Standard message and data rates may apply according to your service plan with your mobile carrier.
   
   Frequency of Messages: The number of SMS messages you receive will depend on your interactions with us and the preferences you set within your account settings.

7. Emergency Situations: a. In case of a medical emergency, call 911 or go to the nearest emergency room. b. The clinic should be notified of any emergency room visits or hospitalizations as soon as possible.
8. Termination of Services: a. The clinic reserves the right to terminate services if a patient engages in disruptive behavior, fails to comply with treatment plans, or violates clinic policies.
9. Changes to Terms and Conditions: a. The clinic reserves the right to modify these Terms and Conditions at any time. Patients will be notified of any changes.
10. Governing Law: a. These Terms and Conditions are governed by the laws of Georgia. Any disputes shall be resolved in the courts of Georgia.
